Record Keeping

A few people have sent in some forms for record keeping. Browse through them and print the ones that will work for your Activity Day group. Each form is in three different formats for your convenience. Most of you will want to choose the pdf file to print from. If you have Microsoft Word on your computer you could download the ms word file and make changes or add the names of your children before printing. If you can't get the form to print with the pdf file or the ms word file then the html file should work on all computers. When you click on a file, make sure that you scroll down through the pages to see all of the forms that each person has contributed. If you have designed any forms that are different from these, please email me and I will add it to this page.
